師范類：往屆畢業(yè)生需準(zhǔn)備以下材料：

1、本人身份證原件和復(fù)印件；

2、師范教育類專業(yè)畢業(yè)證書原件和復(fù)印件；

3、申請人思想品德鑒定表（在職人員由其所在單位提供，非在職人員由其戶籍所在地鄉(xiāng)（鎮(zhèn)）政府或街道辦事處提供）；

4、本人填寫教師資格認(rèn)定申請表；

5、由教師資格認(rèn)定機(jī)構(gòu)指定的縣級以上醫(yī)院出具的體格檢查合格證明；

6、普通話水平測試等級證書原件和復(fù)印件。應(yīng)屆師范生可在其畢業(yè)前的最后一個(gè)學(xué)期，向就讀或擬任教學(xué)校所在地教師資格認(rèn)定機(jī)構(gòu)提出認(rèn)定教師資格申請，申請時(shí)以學(xué)業(yè)成績單代替學(xué)歷證書，思想品德鑒定表由在讀學(xué)校提供。其余要求與往屆生相同。上述材料經(jīng)教師資格認(rèn)定機(jī)構(gòu)審查合格后，方可領(lǐng)取教師資格證書。非師范類：與師范類應(yīng)屆、往屆畢業(yè)生需準(zhǔn)備的材料基本相同。只是在初審合格后，須參加由教師資格專家審查委員會組織的教育教學(xué)基本素質(zhì)和能力考察，并提供教育學(xué)、心理學(xué)考試成績證明。如果符合條件，教師資格認(rèn)定機(jī)構(gòu)將在相應(yīng)的受理期限內(nèi)通知認(rèn)定結(jié)果。

中小學(xué)幼兒園教師資格認(rèn)定工作的通知

各市教育局，有關(guān)高等學(xué)校：

根據(jù)?中華人民共和國教師法?（以下簡稱?教師法?）、?教師資格條例?、??教師資格條例?實(shí)施辦法?和?山東省實(shí)施細(xì)則?（魯教人字?2001?22號）的有關(guān)規(guī)定，現(xiàn)就做好我省2010年中等職業(yè)學(xué)校、中小學(xué)和幼兒園教師資格認(rèn)定工作通知如下：

一、認(rèn)定范圍

幼兒園、小學(xué)、初級中學(xué)、高級中學(xué)、中等職業(yè)學(xué)校教師資格和中等職業(yè)學(xué)校實(shí)習(xí)指導(dǎo)教師資格的申請人員，包括：

（一）各級各類學(xué)校已取得教師資格擬申請其他等次教師資格申請人員；

（二）2010年普通高校應(yīng)屆畢業(yè)生；

（三）其他符合?教師法?規(guī)定學(xué)歷條件的人員。

二、申請條件

（一）未達(dá)到國家法定退休年齡、戶籍或工作單位在我省的中國公民。

（二）要具備?教師法?、?教師資格條例?及其?實(shí)施辦法?規(guī)定的相應(yīng)學(xué)歷。

（三）教育

教學(xué)

1.取得?山東省教師資格認(rèn)定教育學(xué)、心理學(xué)考試合格證?(以下簡稱?合格證?)并經(jīng)教師資格認(rèn)定機(jī)構(gòu)進(jìn)行教育教學(xué)能力測評合格（修學(xué)教育學(xué)、心理學(xué)課程成績合格且參加畢業(yè)教學(xué)實(shí)習(xí)成績合格的普通院校師范類畢業(yè)生可免測）。

2.普通話水平要達(dá)到普通話水平測試二級乙等以上標(biāo)準(zhǔn)，取得相應(yīng)合格證書。

3.適應(yīng)教育教學(xué)工作的需要，具有良好的身體素質(zhì)和心理素質(zhì)，無傳染性疾病，無精神病史，并經(jīng)教師資格認(rèn)定機(jī)構(gòu)組織體檢合格的。

（四）要遵守憲法和法律，遵守教師職業(yè)道德，能履行?教師法?規(guī)定的義務(wù)，經(jīng)申請人所在單位（含就讀學(xué)校）或戶籍所在地鄉(xiāng)鎮(zhèn)人民政府（街道辦事處）思想品德鑒定合格。

三、

（一）教育學(xué)、教育心理學(xué)考試

申請教師資格認(rèn)定人員須到戶籍或工作單位、就讀學(xué)校所在地教師資格認(rèn)定機(jī)構(gòu)報(bào)名參加教育學(xué)、教育心理學(xué)（以下簡稱“兩課”）考試，取得?合格證?。

（二）教育教學(xué)能力測評

取得?合格證?的申請人員參加市、縣（市、區(qū)）教師資格認(rèn)定機(jī)構(gòu)組織的教育教學(xué)能力測評。

（三）體檢

取得?合格證?且教育教學(xué)能力測評合格的申請人員和符合教師資格認(rèn)定特許條件的申請人員，由教師資格認(rèn)定機(jī)構(gòu)按?山東省教師資格認(rèn)定體檢標(biāo)準(zhǔn)及操作規(guī)程?組織體格檢查。

（四）資格認(rèn)定

符合條件的申請人員填寫?教師資格認(rèn)定申請表?，提供身份證件、學(xué)歷證明、?合格證?、?山東省申請教師資格人員體格檢查表?、?普通話水平測試等級證書?及?申請人思想品德鑒定表?，向戶籍或工作單位、就讀學(xué)校所在地的教師資格認(rèn)定機(jī)構(gòu)提出申請。

教師資格認(rèn)定機(jī)構(gòu)受理申請后在30個(gè)工作日內(nèi)作出認(rèn)定結(jié)論。認(rèn)定合格者頒發(fā)?教師資格證書?，按要求將文字材料歸檔保存，并使用“全國網(wǎng)絡(luò)版教師資格認(rèn)定信息管理系統(tǒng)”錄入認(rèn)定數(shù)據(jù)信息。

普通院校師范類畢業(yè)生修學(xué)教育學(xué)、心理學(xué)課程成績合格且參加畢業(yè)教學(xué)實(shí)習(xí)成績合格的，可免測以上

（一）、（二）項(xiàng)測試程序。但學(xué)習(xí)期間缺少教育學(xué)或心理學(xué)合格成績的，仍須參加“兩課”考試并取得?合格證?；缺少畢業(yè)教學(xué)實(shí)習(xí)合格成績的，仍須參加教育教學(xué)能力測評且成績合格。普通高校師范類

應(yīng)屆畢業(yè)生的教師資格認(rèn)定工作，由各高校學(xué)生處或就業(yè)指導(dǎo)中心負(fù)責(zé)統(tǒng)一組織，參加駐地市教師資格認(rèn)定機(jī)構(gòu)的認(rèn)定。教師資格認(rèn)定機(jī)構(gòu)要確保畢業(yè)生離校前向合格人員發(fā)放?教師資格證書?。

四、“兩課”考試

2010年“兩課”考試由我廳統(tǒng)一組織，分中學(xué)（含中等職業(yè)學(xué)校）、小學(xué)和幼兒園3個(gè)層次。申請認(rèn)定中等職業(yè)學(xué)校教師資格，中等職業(yè)學(xué)校實(shí)習(xí)指導(dǎo)教師資格，高級中學(xué)、初級中學(xué)教師資格的人員參加中學(xué)層次考試；申請認(rèn)定小學(xué)教師資格的人員參加小學(xué)層次考試；申請認(rèn)定幼兒園教師資格的人員參加幼兒園層次考試。

考試報(bào)名工作采用網(wǎng)上報(bào)名與現(xiàn)場報(bào)名相結(jié)合的方式進(jìn)行?？忌氃谝?guī)定時(shí)間內(nèi)訪問山東省教師資格認(rèn)定指導(dǎo)中心網(wǎng)站（）公布。

（三）體格檢查

教學(xué)能力測試合格對象和免于測試對象都必須參加統(tǒng)一組織的體檢，體檢人

員名單及體檢工作安排于教學(xué)能力測試結(jié)束一周內(nèi)公布在儀征教育信息網(wǎng)

（.cn），體檢結(jié)果在體檢結(jié)束10日內(nèi)公布在儀征教育信息網(wǎng)，體

檢合格人員在規(guī)定的提交材料時(shí)間內(nèi)提交材料。

（四）提交材料

1、提交材料時(shí)間：公布體檢結(jié)果時(shí)一并公布

2、提交材料地點(diǎn)：儀征市行政辦事服務(wù)中心綜合窗口

3、提交材料種類：

（1）申請認(rèn)定教師資格材料袋封面（現(xiàn)場領(lǐng)取材料袋并粘貼）；

（2）《教師資格認(rèn)定申請表》2份(正反打印)；

（3）身份證復(fù)印件；

（4）戶籍證明材料復(fù)印件；

（5）符合相應(yīng)條件的學(xué)歷證書復(fù)印件（2010年應(yīng)屆非師范畢業(yè)生須提交學(xué)

業(yè)成績證明原件，樣式附后）；

（6）普通話水平測試等級證書原件、復(fù)印件；

（7）思想品德鑒定材料1份；其中在職人員應(yīng)由所在工作單位提供思想

品德證明。應(yīng)屆畢業(yè)生應(yīng)由畢業(yè)學(xué)校所在院系黨組織提供思想品德證明。非在職

人員應(yīng)由其戶籍所在地街道辦事處或鄉(xiāng)級人民政府提供思想品德證明，并由當(dāng)?shù)?/p>

公安部門 提供無犯罪記錄證明；

（8）非師范專業(yè)學(xué)歷提供教育學(xué)、教育心理學(xué)補(bǔ)修合格證原件；無法以學(xué)

歷證書確認(rèn)師范身份者需提供學(xué)歷證書頒發(fā)學(xué)校該專業(yè)的招生考試說明和教育

實(shí)習(xí)檔案復(fù)印件（檔案管理單位需加蓋公章并注明與原件相符）；已具有初中、高中、中職、中職實(shí)習(xí)指導(dǎo)教師資格證書的提供證書復(fù)印件；

（9）粘貼相片頁。

復(fù)印件請統(tǒng)一用a4紙按封面的證明材料順序裝訂好。

（五）證書發(fā)放

經(jīng)認(rèn)定符合教師資格條件的人員名單在儀征教育信息網(wǎng)公布，于7月初到儀

征市行政辦事服務(wù)中心綜合窗口領(lǐng)取教師資格證書。
